Muscle loss on incretin therapy: what the data say

Lean-mass loss is the most cited criticism of GLP-1 drugs. The numbers are more nuanced than either side claims.

The headline figure

Body-composition substudies typically report that roughly a quarter to 40% of total mass lost on incretin therapy is lean tissue. That proportion is broadly similar to what is seen with diet-induced weight loss of the same magnitude.

Why proportion is the wrong frame

Lean mass includes water, glycogen and organ tissue, not only muscle. And absolute lean-mass loss matters more in older adults with low baseline muscle than in a younger person losing a large amount of fat.

What is being tested

Several programmes now pair an incretin with a muscle-preserving agent — myostatin and activin pathway antibodies among them — with the explicit goal of shifting the composition of loss toward fat.
